Care Navigation
Each Connected Care member is paired with a dedicated Patient Navigator, their personal “inside” partner within Emerson Health.
Navigators help employees and individuals:
- Find a primary care provider who fits their needs
- Access specialists quickly
- Coordinate referrals and follow-up care
- Navigate urgent and complex medical concerns
- Connect to Emerson Health services and resources
This is concierge care for the rest of us, delivered at a fraction of the cost of traditional concierge programs.
Seamless Access
Connected Care provides seamless access to a full network of services, including:
- Primary and specialty care
- Three Emerson Urgent Care locations
- Virtual urgent care visits
- Virtual behavioral health through Colony Care
- Occupational Medicine and Employee Health services
- Return-to-work and injury management support
- Clinical affiliation with Mass General Brigham for advanced care access
All services are coordinated within a single Emerson Health medical record.
