Yes. You do not need to have an Emerson Health physician to enroll in Connected Care. Navigators can assist you regardless of your current provider relationships.

Connected Care itself is a navigation and care coordination service. Your Navigator may be able to help identify telehealth options for some services available through your healthcare providers.

No. Connected Care does not provide medical care or replace your PCP or specialists. Instead, it helps you navigate the healthcare system and connect with the right providers and services.

Yes. All personal and health information is handled according to applicable privacy and confidentiality requirements.

Connected Care is not an emergency service. Members should call 911 or seek emergency medical care for urgent or life-threatening situations. Connected Care is available during normal business hours. Navigators will reach out within 1-2 business days responding to requests.

Health insurance helps pay for healthcare services, but it does not typically provide personalized healthcare navigation. Connected Care gives you access to a dedicated Patient Navigator who can help coordinate care, identify providers, assist with appointments, and simplify complex healthcare journeys.
