Emerson Health’s Medical Records Department is committed to safeguarding your health information, ensuring it remains confidential and secure. We uphold your patient rights and comply with all relevant federal and state regulations to protect your medical records.
Record Requests
To obtain a copy of your records:
- print out this form, and
- fax it to 978-287-3652 or
- mail it to the address at the top of the form
Be sure to complete the form, including your date of birth, dates of service being requested, and what the records are being obtained for (i.e., personal, physician visit, legal, etc.). Please include your phone number and the complete mailing address for where we should send the records. Please allow 7 to 10 business days for a standard request to be completed. A fee may be charged for copies of your medical record.
If you need to speak to someone, please call 978-287-3907 between 8:00 AM and 4:30 PM, Monday through Friday. We will mail your records to the address specified on the release of information form.
Important Notes
Massachusetts law requires that we maintain medical records for 20 years following your last visit. These records include laboratory and radiology reports, emergency department visits, outpatient surgeries, and inpatient stays at Emerson Hospital.
- The medical records department cannot access individual physician office records or any Emerson Urgent Care Center records; you must contact that office directly to obtain a copy of your records.
- For radiology records (x-ray, MRI, CT, and other imaging), contact our radiology department at 978-287-3700.
- For all other records, please follow the instructions below.
Birth Certificates
Emerson Hospital will complete and send birth records to the Concord town clerk’s office within ten days of birth. To help us ensure the completion of your baby’s birth certificate on time, please complete the below parent worksheet before the mother and baby are discharged home.
If you need a copy of a birth certificate, call the town clerk’s office in the town you lived in at the time of the birth. You can also contact the Concord Town Clerk’s Office at 978-318-3080 or by mail at Concord Town Clerk, PO Box 535, Concord, MA 01742. The clerk can provide you with a certified copy of the birth certificate. A fee for this service may be charged. Paternity papers may also be completed post-birth at the town clerk’s office.
